Please print out both pages of the appropriate Welcome form, adult or child, the Privacy Policy form, the Missing Appointment Policy Form and the Contact form, fill them out and bring them to your first appointment. If you cannot print these pages, please arrive 15 minutes prior to your appointment to allow time for paperwork.
